Sponsored by the Robert Wood Johnson Foundation, this award honors communities at the forefront of health improvement. Applicants must describe their community, its population health challenges, and how their community became a healthier place to live, learn, work, and play. Up to six $25,000 awards will be announced in early 2013. A brief notice of intent to apply is due by May 31, 2012.
